45-90 lbs.

7, 8, & 9 year olds

 

 

Jr. Pee Wee

60 -105 lbs.

(60-85 lbs. Older Lighter)

8, 9, & 10 year olds

 

11 year old (O\L)

 

 

Pee Wee

75-120 lbs.

(75-100 lb. Older Lighter)

9, 10, & 11 year olds

 

12 year old (O\L)

 

 

C                           CHEER DIVISIONS ARE: Mitey Mite (ages 7,8,9) Jr. Pee Wee (ages 8,9,10 & 11 O\L),

    & Midget (ages 11, 12, 13, 14, & 15 O\L).

 

                ALL OF THE FOLLOWING FORMS MUST BE TURNED IN PRIOR TO AUGUST 1ST, 2010

                         OR YOUR CHILD WILL BE MOVED TO THE WAITING LIST!

1) Parental Consent\Registration Form (Must be filled out by a parent on day of signup)

2) Medical Part 1 (filled out and answered by parent, Must be filled out on day of signup)

3) Medical Physical (dated after Jan 1st, 2010) ( Can be turned in at signup if complete.  Must be turned in by Aug. 1st, 2009, or participant will be moved to waiting list!)

4) Copy of Birth Certificate (Should be turned in on day of signup; but not later than Aug. 1st, 2010)

5) Report Card (4th quarter of 2008-2009 school year); (must be turned in before Aug. 1st, 2010)

6) Recent Photo (should be turned in on day of signup, but not later than Aug. 1st, 2010) PRINT NAME OF PARTICIPANT ON BACK OF PHOTO!

All registration fees are due at the time of sign-up!

*Those put on waiting list may pay when they move onto regular list, NO OTHER EXCEPTIONS!
